OASIS: Chronic Disease Self-Management Program

 Do you have an ongoing health condition and want to take charge of your life? Join this Chronic Disease Self-Management Program (CDSMP). The CDSMP is a six-session peer lead health education program for adults (18+) with any type of ongoing health condition. Each session complements the health care you may already be receiving.

CDSMP workshops will help you:
Feel Better. Increase your energy and get relief from pain, fatigue and difficult emotions.
Take control of your life. Do the things you want to do each day.
Get connected. Have the opportunity to learn from others who have similar health issues.

CDSMP was developed at Stanford University's Patient Education Center and is brought to you through a collaboration between Onondaga County Department of Aging and Youth, The Centers at St. Camillus, The Advocates for Upstate, and OASIS, a program of Upstate Medical University.

You must pre-register.

 Workshop lead by Co-peer leaders certified in CDSMP by Stanford University. Class size is limited to 20. Registration begins after August 26th by calling OASIS 315-464-6555. Free six sessions.
